Service Agreement
This Service Agreement (“Agreement”) is entered into between Coastal Med Logistics, LLC (“Courier”) and the requesting facility or individual (“Client”) for the transport of medical specimens, supplies, and related materials.
By requesting services, the Client agrees to the following terms:
1. Scope of Services
Courier agrees to provide timely, secure transportation of medical items including but not limited to:

Biological specimens (including UN3373 Biological Substance, Category B)
-
Medical supplies and equipment
-
Pharmaceuticals and related materials
All services will be performed in accordance with applicable federal, state, and local regulations, including proper handling, packaging, and transport requirements.
2. Service Types & Priority Levels
Services are offered under the following categories:

STAT Delivery: Immediate pickup and direct transport to destination without stops
-
Same-Day Delivery: Pickup and delivery completed within the same business day
-
Scheduled/Routine Delivery: Pickup and delivery within a pre-arranged time window
-
Temperature Controlled Delivery
-
After Hours Delivery
-
Overnight Delivery
Priority level must be specified at time of request and may affect pricing and response time.
3. Client Responsibilities
The Client agrees to:

Ensure all items are properly packaged and labeled in compliance with applicable regulations (including UN3373 where applicable)

Provide accurate and complete:
-
Pickup and delivery information
-
Contact details for responsible parties
-
Description of contents (as required)
-
Disclose any special handling requirements (temperature sensitivity, fragility, urgency)
Courier is not responsible for delays or damages resulting from improper packaging or incomplete information.
4. Chain of Custody
Courier will maintain documented chain of custody, including:

Pickup confirmation (signature or equivalent)
-
Delivery confirmation (signature or equivalent)
-
Timestamped transport records
Responsibility for the shipment transfers to Courier upon pickup and transfers to the receiving party upon delivery confirmation.
5. Temperature Control
When temperature-controlled transport is required:
-
Courier will utilize insulated containers and cooling materials as appropriate
-
Temperature ranges are maintained to the best of Courier’s ability using non-powered cooling methods unless otherwise agreed
-
Client must specify required temperature range at time of request
-
No guarantee of exact temperature range unless specifically contracted
Courier is not liable for temperature deviations caused by:
-
Improper packaging by Client
-
External environmental conditions beyond reasonable control
-
Undisclosed temperature requirements
6. Delivery Attempts
Courier will make reasonable efforts to complete delivery at the designated location.
If delivery cannot be completed:
-
Cargo will be returned to sending facility and will not be held by the Courier
-
Client will be contacted for further instructions
-
Re-delivery will have to be scheduled as a new job
7. Fees & Payment Terms
Pricing is based on service type, distance, and any additional requirements.
Final pricing will be communicated prior to or at time of service confirmation
Payment terms:
-
Net 30 - unless otherwise agreed
-
The Courier has the right to suspend service for unpaid invoices
-
Late payments may be subject to additional fees or service suspension
-
Wait time for specimen pickup over 15 minutes will incur a $20 fee per each 15 minute increment. This will be billed as a Wait time/detention fee
8. Cancellations
Cancellations must be made prior to dispatch to avoid charges.
If a courier has already been dispatched:

Enroute cancellation: 75% of the service will be charged

Upon arrival at pickup location: 100% of the service will be charged
9. Liability
Courier’s liability is limited to:
-
Direct loss or damage caused by proven negligence
-
Maximum liability shall not exceed the total service fee for the shipment
Courier is not liable for:
-
Indirect or consequential damages
-
Loss due to improper packaging
-
Regulatory non-compliance by Client
-
Delays caused by traffic, weather, or unforeseen circumstances
10. Compliance
Client affirms that all materials provided for transport:
-
Comply with all applicable laws and regulations
-
Are safe for transport under agreed conditions
-
Are properly declared and documented
Courier reserves the right to refuse any shipment deemed unsafe or non-compliant.
11. Insurance
Courier maintains general liability coverage. Additional insurance coverage for high-value items must be requested in advance and may incur additional fees. No guarantee of coverage for contents. High-value items must require declared value.
12. Communication & Notifications
Courier will provide service updates including:

Pickup confirmation
-
Delivery confirmation
-
Relevant status updates as needed
Communication may be conducted via phone, SMS, or email.
13. Independent Contractor Status
Courier operates as an independent contractor and is not an employee or agent of the Client.
14. Governing Law
This Agreement shall be governed by the laws of the State of California.
15. Acceptance of Terms
By requesting and utilizing services, Client acknowledges and agrees to all terms outlined in this Agreement.
